Social Prescribing services

Social Prescribing supports the health and wellbeing of people by encouraging them to connect with groups and services within their community.

The Social Prescribing service offers individuals an opportunity to discuss their health and wellbeing needs with a Social Prescribing Link Worker. Together, they devise a plan to address their needs by engaging with appropriate resources in the community. The individual decides what options they select, whether it is taking up a class, commencing a new hobby or pastime, or joining a local social group. The link worker is available to provide relevant information and to support the individual in making the links with groups, services and activities in the community.

There are Social Prescribing services located in Clonmel and in Nenagh.

Target Groups: Adults aged 18 years and upwards who:

  • Feel lonely, socially isolated and anxious
  • Need support around their mental health and wellbeing
  • Are new to the community
  • Are affected by a long-term health condition
  • Are under stress due to bereavement, unemployment, retirement, family difficulties, or other life-changing developments

Eligibility:

  • Re. service in Clonmel, adults resident in Clonmel and immediate environs.
  • Re. service in Nenagh, adults resident in Nenagh and North-West of the county (including Newport, Borrisokane, and Cloughjordan)

Referral: Self-referral; referral by GP or other healthcare professionals; referral by community services/agencies
